Keep track of the worst of your symptoms so you don't forget to mention them.
This Journal is filled with everything I wish I had whilst undergoing treatment, from uplifting affirmations to mood squares and things to look forward to.
Welcome to your 'Forget Me Not' Journal.
When I was undergoing treatment for breast cancer, I searched for something where I could put important phone numbers, appointment reminders, positive thoughts, and somewhere to keep track of my symptoms. Unfortunately, I was unable to find such a thing. Hence, the reason for this Journal.
Important Phone Numbers
This section doesn't need to contain just medical numbers. You can use it for the number of your favourite takeaways or a friend who keeps ridiculous sleep schedules and may be awake in the middle of the night if you need them. Use it for any contact number that is important to you.
Throughout the journal, you will find uplifting thoughts, poems and ideas; I have even included a couple of no-hassle recipes
Don't Forget Me
In this section, you can list anything you wish to discuss with a medical professional at your next appointment. I found that by the time I spoke to an oncologist after each treatment, the side effects were no longer impacting my life as much, so everything was okay because, at that time, it was.
Diary
You will find an 18-month diary, which includes a month and a week to view, with a space for positive thoughts or something you plan to do the following week, even if it's something as simple as a drive-thru coffee with a loved one.
Plans For The Future
Use these pages to make plans, whether for watching a film at the cinema, a meal out with family or friends, or a foreign trip.
Mood Squares
It is time to get out your coloured pencils again. Colour a square for each time your mood changes; you decide what colour each mood is.
Notes
Lined sheets for any notes that you want to make.
